Bias linked to race, sex, and socioeconomic status is already well known, but a consistent and automatic testbed for measuring it is missing. To fill this gap, this paper presents AMQA -- an Adversarial Medical Question-Answering dataset -- built for automated, large-scale bias evaluation of LLMs in medical QA. AMQA includes 4,806 medical QA pairs sourced from the United States Medical Licensing Examination (USMLE) dataset, generated using a multi-agent framework to create diverse adversarial descriptions and question pairs. Using AMQA, we benchmark five representative LLMs and find surprisingly substantial disparities: even GPT-4.1, the least biased model tested, answers privileged-group questions over 10 percentage points more accurately than unprivileged ones. Compared with the existing benchmark CPV, AMQA reveals 15% larger accuracy gaps on average between privileged and unprivileged groups. Our dataset and code are publicly available at https://github.com/XY-Showing/AMQA to support reproducible research and advance trustworthy, bias-aware medical AI.","url_abs":"https://arxiv.org/abs/2505.19562v1","url_pdf":"https://arxiv.org/pdf/2505.19562v1.pdf","source":{"archive":"pwc-archive (Hugging Face), CC BY-SA 4.0","snapshot":"2025-07-28","licence_url":"https://creativecommons.org/licenses/by-sa/4.0/legalcode","row_kind":"abstracts"},"code_links":[{"paper_slug":"amqa-an-adversarial-dataset-for-benchmarking","repo_url":"https://github.com/xy-showing/amqa","is_official":1,"mentioned_in_paper":1,"mentioned_in_github":0,"framework":"none","reach":null}],"tasks":[{"task_slug":"benchmarking","task_name":"Benchmarking"},{"task_slug":"medical-diagnosis","task_name":"Medical Diagnosis"},{"task_slug":null,"task_name":"Medical Question Answering"},{"task_slug":"question-answering","task_name":"Question Answering"}],"methods":[{"method_slug":"absolute-position-encodings","method_name":"Absolute Position Encodings"},{"method_slug":"adam","method_name":"Adam"},{"method_slug":"attention","method_name":"Attention"},{"method_slug":"bpe","method_name":"BPE"},{"method_slug":"dense-connections","method_name":"Dense Connections"},{"method_slug":"dropout","method_name":"Dropout"},{"method_slug":"gpt-4","method_name":"GPT-4"},{"method_slug":"label-smoothing","method_name":"Label Smoothing"},{"method_slug":"layer-normalization","method_name":"Layer Normalization"},{"method_slug":"linear-layer","method_name":"Linear Layer"},{"method_slug":"multi-head-attention","method_name":"Multi-Head Attention"},{"method_slug":"position-wise-feed-forward-layer","method_name":"Position-Wise Feed-Forward Layer"},{"method_slug":"residual-connection","method_name":"Residual Connection"},{"method_slug":"softmax","method_name":"Softmax"},{"method_slug":"transformer","method_name":"Transformer"}],"datasets_introduced":[],"methods_introduced":[],"results":[],"syntology":{"syntology_url":"https://syntology.ai/paper/2505.19562","atlas_url":"https://app.syntology.ai/?focus=2505.19562","mcp":null,"developers":"https://syntology.ai/developers"},"arxiv_metadata":null,"syntology_extracted_results":null}
